Understanding Achilles Tendinopathy

Achilles tendinopathy is persistent pain in the tendon that connects your calf to your heel. It often shows up after you bump up mileage, add hill work, or increase plyometrics. It can make simple things—jogging, single-leg hops, or landing from a jump—hurt more than they should.

This isn't always about complete rest. We use progressive loading protocols — not blanket rest — to rebuild the tendon’s capacity. That means targeted strength work, smarter training, and hands-on care so you can keep doing the activities you love while your tendon gets stronger.

If you’re worried this means you’re done with running, hiking, deadlifts, or dance, come get a professional evaluation. We’ll identify what’s driving the pain and give you a clear plan — not guesswork.

Common Symptoms

  • Achilles pain localized above or at the heel that worsens with activity
  • Stiffness or tightness in the tendon first thing in the morning or after sitting
  • Pain that flares when you increase distance, speed, hill work, or jumping
  • Localized swelling, tenderness, or a thickened feeling in the tendon
  • Difficulty pushing off during running, jumping, or single-leg work

Common Causes

  • Sudden increase in training load—mileage, speed, hills, or plyometrics
  • Insufficient calf or ankle strength to handle your sport’s demands
  • Training on hard surfaces or with inadequate footwear
  • Poor movement patterns or limited ankle mobility that overload the tendon
  • Tendon tissue changes from repeated stress and inadequate recovery

How We Treat Achilles Tendinopathy

We focus on progressive, strength-based care delivered in 60-minute one-on-one sessions with a Doctor of Physical Therapy. You won't get a 10-minute check-in or a generic exercise sheet. Our approach is hands-on, practical, and built around the 3-Phase Bulletproof Roadmap: manage symptoms and load, rebuild tendon capacity with progressive loading, then return-to-activity and bulletproofing so you can train with confidence.

01

Comprehensive 60-minute Evaluation

You get a full movement and load-capacity assessment with Dr. Sujaya. We look beyond the tendon—calf strength, ankle mobility, gait, training history, and load management. Expect hands-on testing, clear explanations, and a plan that fits your goals (trail runs, deadlifts, dance, or pickleball).

02

Phase 1 — Symptom Control & Load Strategy

We never default to ‘stop.’ Instead we teach you how to modify activity so pain stays manageable. Early work focuses on reducing pain spikes, restoring movement, and teaching a pain-safe load strategy so you can keep training in a controlled way.

03

Phase 2 — Progressive Tendon Loading & Strength

This is the heart of our work. Guided, progressive loading builds tendon capacity and calf strength. Sessions are individualized and progress based on how your tendon responds. We combine hands-on treatment, targeted strengthening, and training integration so gains transfer to your sport.

04

Phase 3 — Return-to-Activity & Bulletproofing

We move you back into real training—hill repeats, tempo runs, jumps, or dance sequences—on a clear ramp-up plan. The goal is to reduce flare-ups and improve tolerance for the specific demands you care about.

05

Ongoing Coaching & Training Integration

You leave with a 3-Phase Bulletproof Roadmap and actionable milestones. We coordinate with your coach or provider if needed. Follow-up visits reassess progress and adjust the plan so you keep moving toward your goals.

What to Expect

  1. 1First visit: a 60-minute, hands-on evaluation. We review your history, observe movement, and test load tolerance. You get an explanation of what’s likely driving the pain and a tailored plan.
  2. 2Early visits: focused on symptom management, hands-on care, and teaching a load strategy that matches your training. You’ll get targeted exercises and practical modifications—modify, don’t stop.
  3. 3Middle visits: progressive strength and tendon-focused work. We advance exercises based on your response and begin sport-specific loading so improvements carry over to running, lifting, or dance.
  4. 4Later visits: guided return-to-activity sessions and education on preventing recurrence. We set milestones, monitor soreness responses, and hand you a plan to keep training safely.
  5. 5Throughout: 1:1 time with a Doctor of Physical Therapy. Regular reassessment keeps the plan aligned with your goals. If imaging or referral to another provider is needed, we’ll discuss and coordinate.

Why Conquer PT

Frequently Asked Questions

Do I need to stop running or training?

Not usually. We use a modify-don't-stop strategy. That means adjusting volume, intensity, or specific drills so you can keep training while the tendon gets stronger. A professional evaluation helps set safe limits.

Will you order imaging (MRI or ultrasound)?

Imaging can be useful in some cases, but it isn’t always required. We will recommend further testing or coordinate with your physician when it would change management or rule out other issues.

How many visits will I need?

It depends on your history, training load, and goals. We don’t promise a specific number up front. Expect a clear plan with milestones and regular reassessment so you know how you’re progressing.

How do I get started?

Personalized, one-to-one care with a clear roadmap for progress.

What if my tendon is very painful or swollen?

We’ll help you manage pain with load strategies and hands-on care. If swelling, severe pain, or signs of a different problem are present, we’ll recommend further evaluation or refer you to the appropriate provider.

Will I need surgery?

Most people don’t need surgery. We don’t perform surgeries in clinic. If surgical evaluation becomes appropriate, we’ll discuss options and coordinate care with your medical team.
