Understanding Sciatica

Sciatica is the name for pain, tingling, or numbness that travels from your low back into your buttock and down one leg. It can make running, sitting, sleeping, and even lifting feel impossible.

You don't have to stop the activities you love. We evaluate what may be compressing or irritating the nerve and create a clear, strength-focused plan so you can modify—not stop—your training while the root issue is addressed.

Common Symptoms

  • Sharp, burning, or electric pain radiating from the low back through the buttock and down one leg
  • Numbness, tingling, or pins-and-needles in the thigh, calf, or foot
  • Pain that worsens with sitting, bending forward, coughing, or certain movements
  • Weakness or a sense of giving way in the hip, knee, or ankle during activity

Common Causes

  • A bulging or herniated disc that irritates the sciatic nerve
  • Nerve compression from tight muscles, especially the piriformis or gluteal muscles
  • Spinal degeneration, bone spurs, or narrowing of the spinal canal (stenosis)
  • Poor movement patterns or weakness that overload the low back and nerve pathways

How We Treat Sciatica

We don’t do 10-minute visits or cookie-cutter stretch sheets. You get a doctor-led, 60-minute, one-on-one session focused on finding what’s driving your flare-ups and building lasting strength around it.

01

Comprehensive movement evaluation

A full 60-minute assessment looks at your spine, hips, pelvis, and how you load through running, lifting, or daily movement. We identify movement faults, weakness, and the likely sources of nerve irritation so you stop guessing.

02

Hands-on care and symptom control

Targeted manual therapy and techniques (including dry needling when appropriate) relieve pain and calm nerve sensitivity so you can move without fear. This is paired with movement you can actually use.

03

Strength-based root-cause training

We build progressive strength and coordination in the exact movement patterns you need — deadlifts, trail runs, hikes, dance turns, or quick pickleball lunges — so the nerve is less likely to flare.

04

Modify, don't stop

We adapt your training so you keep progressing. You’ll get activity-specific modifications so you maintain fitness while the root issues improve.

05

3-Phase Bulletproof Roadmap

Your plan follows three clear phases: calm and control, build strength and movement quality, then bulletproofing and sport-specific return. Each phase has measurable milestones so you know where you’re headed.

06

Doctor-led guidance and coordination

Dr. Sujaya Deodhar oversees your care. We remove insurance hurdles, coordinate with your providers when needed, and give straight answers so you can choose the right path forward.

What to Expect

  1. 1First visit: a 60-minute, one-on-one evaluation with the Doctor of Physical Therapy. We review your history, watch you move, and identify what may be compressing or irritating the nerve.
  2. 2Immediate plan: short-term strategies to reduce pain and nerve sensitivity so you can sleep, sit, and do modified training without constant flares.
  3. 3Progression: focused strength and motor-control work added each week. We prioritize the exact movements you care about — runs, deadlifts, hikes, dance, or pickleball quickness.
  4. 4Ongoing: periodic re-checks and milestone reviews. We adjust load, add sport-specific drills, and prepare you for full return while minimizing setbacks.
  5. 5If imaging or a medical consult is needed, we’ll explain why and help you navigate next steps. We encourage a professional evaluation before major decisions like injections or surgery.

Frequently Asked Questions

What exactly is sciatica?

Sciatica describes pain or altered feeling that follows the path of the sciatic nerve — from the low back into the buttock and down the leg. It’s a symptom, not a single diagnosis, and can come from disc issues, muscle compression, or other spine problems.

Will physical therapy fix my sciatica?

Physical therapy aims to reduce pain, calm nerve sensitivity, and address the movement and strength problems that allow flare-ups. We can’t promise outcomes for any individual, but a targeted, progressive plan often reduces symptoms and helps you return to activity.

Can I keep running or lifting?

Usually yes — but with a plan. We focus on modifying load and technique so you maintain fitness while avoiding movements that amplify nerve irritation. The goal is to keep you active, not force you to stop.

Do I need an MRI?

Not always. Imaging can be helpful in certain cases. We evaluate your symptoms and functional limitations and will recommend imaging or a medical referral only if it will change the plan of care.
